We are a GP practice based in Perth, as part of our telemedicine initiatives, we now offer Australia wide specialist referral services online. If you need to see a specialist anywhere in Australia and you require a referral from a GP, please contact us (email: specialistreferral@gmail.com) with the following information, and we will send you the referral via fax, email or by post (your choice). You will receive the referral on the same day (fax or email only). The fee is $10.
You need to provide the following information for us to assess your need to see an Australian specialist in order to comply with the Health Insurance Regulations.
1. Your details (name, DOB, address, phone no. and Medicare no.)
2. Your preferred specialist details (name, the doctor specialty, address, phone and fax no.)
3. Your symptoms (please be as comprehensive as you can, for example what, where, how, when plus any test or scan results that you may have)
4. Your current medications (with dosage and frequency), any allergies to medication, any past medical/surgical history (hypertenstion, diabetes, operation etc)
